I've had my Goethal's belt about a month now and so these are initial impressions.

It is very well made. In fact, it is a triumph of craftsmanship over design. For example, the assembly of the tube to the front shield involves an additional bent bar which is welded to both. More careful shaping of the shield would have avoided the need for this piece (I think that MM had his belt break at this point).

While it is more comfortable to wear than the Tollyboy (aka William Jones, Access Denied) design for those of us who carry more before them than they should, there are a number of places where it should be improved for greater comfort.

Locking Plate
The bottom of the locking plate is joined to the penis tube by a bent bar, welded to both. There is a gap between the bar and the tube into which one's flesh can get painfully trapped. I filled this with quick setting epoxy putty.
Anus Ring
The foam padding here is quite rough and can severely irritate the buttocks. I solved this by wrapping the ring with PTFE tape (used in the UK by plumbers for sealing pipe threads) which has the additional benefit of making it much easier to clean.
Coccyx Bar
This is not padded and can abrade the crease between the buttocks. I put a thin layer of foam on the inside and wrapped it in the PTFE tape. It is now much more comfortable.
Waist Strap
The front ends of the waist are unpadded. These can graze the belly and can cause folds to get trapped between the belt and outer clothing. I haven't found a reliable solution to this as yet.
Drain Holes
The inside edges of the drain holes are quite sharp and, as the penis expands and contracts, the tip can rub against them causing quite a sharp pain. Filling the top row of holes cured the problem for me, but I haven't found a good material to use yet.

Wearing it has been interesting. The first time I had not made any of the modifications and after a couple of hours and a short walk was in quite a lot of pain. I put the belt aside for a week or so while the buttocks healed and made the first two modifications. A few more attempts for a few hours at a time brought greater success. Bending the waist band upwards at the sides improved comfort and, in a way that I haven't worked out, reduced pressure on the pubic bone. The testicles are forced out around the sides of the penis tube and prevent one from closing ones legs. This is only a minor problem, but movement can require a certain amount of additional planning. I can now wear it with an acceptable level of discomfort (one should be aware of these things shouldn't one) for twenty-four hours at a time.

Washing is possible, bathing or showering I haven't yet attempted. Cleaning inside the penis tube is just about impossible, even when it is not being worn. I think some holes at the top to allow water to be run through would have been a good idea.

I've got a Cable Clip from The Power Exchange and have had a Tollyboy in the past. My overall recommendation would be the KTB for daily use out of the house, as it really discourages inappropriate desires and outside temptations, and the Goethals for overnight and weekends, since it can create a high level of frustration by permitting limited arousal.
